Suggestions for linking 5 offices

Anyone have any suggestions for linking 5 different offices together other than the obvious ISDN, T1, Satellite? Distance is too great for microwave or wireless. DSL and Cable is not available in the area.
The connection would be used for a doctor's office for scheduling.

Take a look at Symantec PCAnywhere at http://enterprisesecurity.symantec.com/products/products.cfm?productID=2 and use it with a dial-up. It's rather crude, but there aren't many options left in your scenario. Larry
